Full Job Description

Job Description The post holder will be working as part of the wider team serving the surgeries at North Elmham, Swanton Morley and Toftwood Medical Centre. With support from the Partners and other Nurse Practitioners, the post holder will provide care for the presenting patient from the initial history taking, clinical assessment, diagnosis, treatment, and evaluation of their care. They will demonstrate safe, clinical decision-making ensuring that they always act within their professional boundaries. They will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients.

The post holder will provide support to the Clinical Manager on clinical audit, research, the development of practice clinical guidelines, procedures and protocols, education programmes and quality initiatives such as QOF and CQC (Care Quality Commission) inspections. Person Specification The successful candidate will be: - NMC (Nursing and Midwifery Council) (Nursing and Midwifery Council) Registered with current and up to date Revalidation and Registration. - Qualified Nurse Practitioner with an appropriate level Degree/Masters. - Hold a valid Non-Medical Prescriber qualification.

  • Hold a clean and valid Driving Licence. - Have experience with Minor and Acute presenting illness with advanced level decision making skills. - Hold the ability to work autonomously and have good decision-making skills. - Primary Care experience is preferred but not essential.

  • Knowledge of working with System 1. - Knowledge of working to QOF (Quality Outcomes Framework) requirements. - Ability to follow protocol and procedural practice and National guidelines - Excellent team working and communication skills - Flexible and multiskilled with practice based and home visiting patient care and assessment. DBS The post would be subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and the provision of a satisfactory DBS check.


  • Clinical and Professional Requirements: - Work autonomously and collaboratively with other members of the wider MDT (Muti Disciplinary Team) in the provision of high standards of patient care. - Demonstrate and utilise experience, knowledge, and skills. - Demonstrate the ability to assess patients with undifferentiated, undiagnosed problems through obtaining a clear and concise history with a thorough clinical examination. - Use decision-making skills to formulate a differential diagnosis.

  • Demonstrate the ability to order and interpret necessary diagnostic investigations. - Effectively communicate to both patient and the wider MDT. - Prescribe within professional boundaries and competence. - Maintain accurate and detailed patient records.

  • Refer appropriately to the wider Clinical Team and Secondary Care where appropriate - Act Professionally always, and within the NMC Code of Conduct. - Be prepared to support and teach others within the Clinical Team.
