Full Job Description

There has been an exciting opportunity become available for a passionate and proactive individual who wants to develop their career in a community learning disability team.

Under the guidance of a qualified practitioner, you will implement care plans and provide support for service users within the community learning disability setting, and report on the service users wellbeing.

The post holder will work flexibly in a range of settings, and in partnership with other agencies, carers/family members and the wider multi-disciplinary team.

There will be opportunities for continued professional development, including training around Positive Behaviour Support (PBS) and Trauma informed care.

Main duties of the job

Working as part of the Intensive Support Team, you will undertake short term, focused work with the aim of preventing a person going into crisis and being admitted to hospital.

You will provide advice and training to the wider community team, families and care providers to ensure support plans are in place, and work with other health agencies to support successful discharges from hospital into the community.

There will be an expectation that you will travel to work with people across the footprint of the Trust and out of area placements as well.

CWP provides health and care services for local people, including mental health, learning disability, community physical health and all-age disability care - including the provision of three GP surgeries.
