Full Job Description

22.5 hours per week. The Community Nursing Team provide nursing care to patients in their own homes in the beautiful setting of west Devon. The Community Nursing Service sits within a health and social care team so the work involves working with an integrated approach to care, making every contact count. The team work closely with GP partners, local Hospice's and Voluntary Partnerships. Its an exciting and vibrant opportunity to join the team. This is a great opportunity to gain skills working in the Community Nursing Service., The main duties of the role are to provide nursing care for housebound patients in their own homes. To promote self care and preventative approaches to ill health with focus on wellbeing the emphasis being of preventing hospital admissions and living well.
