Lead Pracititioner (Adults)

• Proactive leader with robust managerial skills.


• Adept at handling complex cases in social work, particularly in an adult social care context.


• Proficient in the development and implementation of quality assessments and care plans for vulnerable individuals.


• Committed to improving outcomes through individualised care packages.


• Focused on maintaining and enhancing the reputation and standing of the Service and the Council.


• Experienced in social work and case management with the ability to support colleagues and lead teams.


• Capable of assuming the responsibilities of a Team Manager as necessary.


What the job involves:

• Providing essential support to Team Managers in managing, developing, and leading a social work team.


• Creating top-quality assessments and support plans tailored to the needs of vulnerable individuals.


• Commissioning individualised care packages for improved outcomes.


• Playing a vital part in upkeeping the Service and the Council's reputation.


• Assisting social workers in managing complex case loads.


• Taking on an agreed upon workload of complex or specialist cases depending on local circumstances.


• Stepping into the Team Manager's role when necessary and deputising in their absence.
