Full Job Description

To work under the direction of the Centre Manager in order to provide appropriate support for our tenants, enabling them to live as independently as possible in a secure, homely environment.
Accountable for: Supervision of staff on shift. Keyworker for maximum of 2 tenants.
Accountable to: Centre Manager.

As Team Leader you will lead a team of staff on a daily basis to ensure that tenants' needs are met.

Responsibilities as Team Leader in addition to providing support to all tenants

  • Provide support to the Centre Manager for the efficient running of Raphael Centre

  • Ordering and delivering of all medication, ensuring all medication is given at the correct times

  • Leading a team of staff on a daily basis to ensure all needs of residents are being met

  • Reporting all and any concerns to the Centre Manager

  • Dealing with rota management

  • Liasing and being involved in multi-agency meetings

  • Deputising for the Centre Manager, when needed


  • All Raphael staff should
  • Provide practical, emotional and intellectual support to all tenants at Raphael Centre, including support for spiritual and social needs.

  • Develop good communication skills with the tenants, which may include learning to use other forms of communication

  • Assist tenants, where necessary with self-help and life skills with the aim of enabling them to carry out such tasks on their own in the future e.g. shopping, assistance with laundry, and preparing meals and drinks.

  • Help tenants meet their personal care needs sensitively and appropriately to a high standard.

  • Enable individuals to achieve independence as far as possible in all areas of their life by providing support and appropriate information on training and counselling.

  • Create opportunities for social and leisure activities with tenants including introducing individuals to a range of facilities and amenities in the local neighbourhood and wider community.

  • Complete and maintain written records as required including care notes.

  • Report any concerns about any aspect of the tenants' emotional and physical health to the tenant's Key Worker or the Centre Manager

  • Support tenants to actively contribute to the running of their own house e.g. keeping their rooms clean and to keep the communal areas to a high standard of cleanliness.

  • Maintain a homely atmosphere.

  • Support tenants to shop for and cook meals for the house as required.

  • Report any untoward occurrences, accidents or complaints immediately according to the agreed procedure.

  • Report any maintenance and repair tasks in the house to the Manager

  • Promote the values underpinning the provision of support

  • Promote the importance of individuality, identity, rights, choice, privacy, independence, dignity, respect and partnership.

  • Actively promote equal opportunities and challenge prejudice or discrimination; through formal and safeguarding procedures where appropriate.

  • Be familiar with and operate within Raphael Centre policies and procedures and any relevant contractual requirements.

  • When working unsupervised, have due regard to on call procedures, and take appropriate responsibility for the proper running of the shift/session/activity and manage own workload.

  • In line with the Health & Safety Policy, be responsible for matters relating to personal health and safety and the health and safety of others. To ensure that current health and safety regulations are met.

  • Be fire risk aware and expert in the house's evacuation and fire drill procedure.

  • To attend staff meetings, as required.

  • To participate in regular supervisions and annual appraisals.

  • To be responsible for your continual personal development and undertake training as required by management, for example First Aid, Fire Risk Management.

  • Any other duties as reasonably required & directed by the Manager.


  • In addition, the Key Worker is responsible for the overall well-being of the tenant(s) they are working with
  • Working with your tenant(s) and their relatives/carers to meet the tenant's physical, emotional and social needs in a way that puts them at the centre of our service, respecting their rights, choice, dignity and diversity.

  • Leading by example, modelling ethical behaviour in the workplace, and supporting Raphael Centre's other dedicated staff to deliver a quality service.

  • Working in line with best practice and meeting appropriate regulatory, contractual and legal requirements.


  • Essential Key Worker duties include
  • Working in partnership with your tenant(s) to produce, follow and update their person-centred plan.

  • Developing good communication skills with the tenants and their other carers.

  • Maintaining written records as required including care notes.

  • Promote individuals health & wellbeing by ensuring their primary healthcare needs are met (medical, dental, optical etc.)

  • Support tenant(s) in order repeat prescriptions and making appointments, if necessary

  • Maintaining your tenant(s) personal hygiene as needed including liaising with their domiciliary carers.

  • Providing a full range of emotional support when needed.

  • Supporting your tenant(s) to manage their finances and planning for things they need.

  • Supporting the individual to access activities they wish to attend including regular activities and theatre trips, gigs and trips.

  • Supporting tenants to do therapeutic holistic activities including bushcraft and arts and crafts.

  • Support your tenant(s) in planning holidays, outings, celebrations & future plans.

  • Supporting tenants to maintain their rooms to ensure they remain clean and safe.

  • Supporting tenants with their food preparation/nutritional needs.

  • Working in a team environment (including attending meetings).

  • Supporting tenants to report any complaints or suspected abuse.

  • Act on any concerns raised by colleagues, relatives, carers or outside agencies, investigate and escalate where necessary.

    Raphael House opened its doors to people with learning difficulties in 1970. Since 2002, Raphael House has been a Supported Living Home for nine tenants with learning difficulties.

    Each tenant has their own weekly timetable with activities and jobs varying from colleges, day centres, and city farms as well as voluntary and paid employment. Every encouragement is given to tenants in forming friendships, being self-motivated in pursuing work and leisure activities and enjoying the wide range of facilities available in and around Bristol. We aim to support within a caring environment the individual growth of each tenant in a way that encourages self-fulfilment and community involvement.

    Tenants are supported according to their own individual needs and staff always maintain a sensitive awareness for the current and changing needs of each individual. Much thought comes from the staff team, working alongside the wishes of the tenants, when finding ways in which support can best be offered, how much or little staff involvement is needed and what will be most beneficial to the individual.

    We have a small dedicated staff team which we feel results in greater communication and understanding, and each tenant has their own key worker to offer greater tailored support.

    There are generally at least two members of staff working during the day, and one sleeping-in overnight.

    Options for the tenants include weekly sessions of bushcraft, arts and crafts, meditation and yoga as well as preparing and cooking the daily meal.

  • A recognised Social Services, Health, Education or Community Work qualification(preferably equivalent to Level 5) Desirable
