Full Job Description

To provide initial support to patients through a robust assessment process before making a decision based on gathered evidence to refer to an appropriate professional.

Responsible for inputting accurate core data onto patient record data system, providing advice and signposting to professionals, partner agencies, families, carers and service users. Ensuring a timely and efficient turnaround of appointments

Responsible for coordinating assigning sent by GP's, ensuring adequate capacity in line with service demand and protocols and pathways within the practice.

To carry out all duties in a professional manner and adhere to all Readesmoor's protocols, policies and guidelines at all times

Key duties and responsibilities:

· To work closely and alongside the GP/Clinicians

· Prioritising the GPs workload

· Continuous monitoring of GP lists

· To have excellent communication skills, with the ability to communicate in writing and verbally to a wide range of providers and users of the services, including patients; carers; voluntary; social; primary and secondary care providers

· Checking of daily appointments available and who

· Booking of all LD/MH/dementia reviews

· Regular communication with patients including telephone calls, letters, and texting, to confirm appointments and respond to ad hoc queries

· To be responsible for own performance management through self-management, delivery of goals and tasks set, delivery of contractual requirements and reporting progress

· To collaboratively develop, implement and support effective and flexible systems and structures that are responsive to the needs of service users with different levels of risk, complexity and strengths

· Attend MDT / MH / vulnerable patients / safeguarding meetings

· Reviewing high risk 'red' patients / frail patients

· Help with complaints management

  • Identifying the risks involves in work activities and undertaking such activities in away that manages those risks

  • Making effective use of training to update knowledge and skills

  • Using appropriate infection control procedures, maintaining work areas in a tidy and safe way free from hazards.
